THE DRIVING IRON — PARTICIPANT LIABILITY WAIVER AND RELEASE OF ALL CLAIMS

Please read this waiver carefully before participating. By checking the consent box, you agree to all terms below.

1. Assumption of Risk

I understand and acknowledge that participation in golf simulator activities involves inherent risks, including but not limited to: physical injury from golf equipment, balls, or simulator components; slipping, falling, or tripping in the activity area; and any other risks associated with golf activities. I voluntarily assume all such risks.

2. Release of Liability

In consideration of being permitted to participate, I hereby release, waive, discharge, and covenant not to sue The Driving Iron, its owners, operators, employees, agents, volunteers, and representatives (collectively "Released Parties") from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or death that may be sustained by me during or in connection with this activity, whether caused by the negligence of the Released Parties or otherwise.

3. Indemnification

I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or cost they may incur as a result of my participation in this activity.

4. Medical Authorization

In the event of an emergency, I authorize the Released Parties to seek medical treatment on my behalf. I acknowledge that I am physically fit to participate and have no medical conditions that would prevent safe participation.

5. Personal Property

I acknowledge that The Driving Iron is not responsible for any loss, damage, or theft of personal property during the event.

6. Acknowledgment

I have read this waiver and fully understand its terms. I understand that I am giving up substantial rights by signing it. I have signed it freely and voluntarily without any inducement, and intend this to be a complete and unconditional release of all liability to the greatest extent allowed by law.

This waiver is governed by the laws of the State of California.
